Pharmacy Benefit Managers (PBMs) Alaska: Myth vs. Fact

Blaming PBMs for high drug prices is factually incorrect and a disservice to Alaska residents. PBMs are the only entities in the drug supply chain that bring down costs for plan sponsors and patients. That is why all types of insurance plan sponsors (including Alaska businesses, municipalities, and the State of Alaska itself) contract with PBMs to manage their prescription drug benefits.

Limits ability to utilize affiliate/preferred pharmacies, prohibits ability to use any tool to direct individuals towards the most cost effective pharmacy option, requires any pharmacy to be in-network if it accepts network  participation terms and conditions, limits ability to establish accreditation  standards for network participation, requires all pharmacies be reimbursed for the dispensing of a drug at no less than the national average drug  acquisition cost for the drug plus a dispensing fee at least equal to the Medicaid rate, prohibits any requirement to dispense specialty self-administered drugs from specific specialty pharmacy, prohibits spread pricing.      

These provisions apply to municipalities offers a group health care insurance plan covering municipal employees, including by means of self-insurance, the municipal health care insurance plan, including the administration and management of pharmacy benefits under the plan.

Imposes a pharmacy reimbursement mandate that PBMs reimburse for the dispensing of a drug at NADAC plus a dispensing fee equal to that of the Medicaid fee, strips network design capabilities by allowing  any pharmacy be in-network and available to patients, prohibits any requirement that a specialty drug that is to be clinician administered be dispensed at a specialty pharmacy that is best equipped to handle.  These provisions would apply to municipalities that offer insurance plans, including by means of self insurance.
